Detect, counsel and follow up on PWSN individual cases identified during psychosocial activities or referred by others.
Conduct psychosocial assessment of vulnerable cases, including environmental, emotional, behavioral, and social factors as well as resources and strengths that impact the individual well- being.
Identification of Cases according to PWSN Criteria
Conduct home visits to assess the living conditions of the cases.
Refer cases internally for legal assistance, psychological counseling or financial assistance when needed.
Coordination with the main service providers in the area of intervention for prompt referral, follow up and information sharing.
Respond to specific need when identified and ensuring is support available to them as required.
Providing emergency assistance and basic emotional support for urgent cases.
Formulating templates and formats to maintain data of PWSN following UNHCR’s protocols
Managing, files, archive specific PWSN intake forms for cases profiling, referral, excel Sheets of Data and Preparing daily, weekly, monthly, quarterly and annual reports, ensuring confidentiality, accuracy and timeliness.
Monitor and follow up of the outcome of the material assistance provided by other partner agencies and services providers.
Established cases management systems and ensuring case follow ups
Organizing Individual Case Management Coordination meetings with the UNHCR and other partners to discuss cases being handled by the psychosocial counselor and the team.
