Compile, organize and maintain accurate records of beneficiaries referred to the OTP/SFP sites by documenting all the essential information in a hard-copy according to the standard OTP/SFP recording format and standards of the ACF.

Actively plan, arrange, record and report various budgeted and non-budgeted meetings with key stakeholders as asked by concerned project management in line with Project TORs.
Actively suggest, arrange, record and report capacity building sessions for key stakeholders as asked by concerned project management in line with Project TORs.
Address and solve problems (if any) faced by female social mobilizer in community by actively involving male community activists/members.
Disseminate among target community information regarding availability of various health services in the relevant health facility of the target area.
Develop close coordination and liaison with health facility staff (Incharge, LHV, EPI and National Program staff) for ensuring maximum results and coverage and developing linkages.
Ensuring that a strong referral mechanism is in place from community to health facility and health facility to tertiary care level facilities.
Attend all meetings with related stake holders at all levels.
Report immediately any untoward incidence to Team Leader/Project Coordinator.
To maintain proper records of supplies, inventory and recording tools.
Make sure that the vehicle (if available) is properly used according to the guidelines.
Maintain record of activities and report regularly to Team Leader/Project Coordinator.
Make sure that all reports are generated and submitted to office on time.
To perform any other relevant duty assigned by the Project Coordinator/Team Leader.
