PROJECT OVERVIEW

The Project aims to enhance immunization activities in high-risk union councils of selected districts through community engagement and strengthened service delivery, including primary healthcare and the Expanded Program on Immunization (EPI). The project focuses on deploying mobile health units, identifying zero-dose and defaulter children to improve vaccine coverage and reduce vaccine-preventable diseases in areas with low immunization rates. Collaborating with key stakeholders such as the EPI Program, health departments, Civil Society Organizations (CSOs), Community-Based Organizations (CBOs), and local communities, the initiative emphasizes raising awareness, ensuring equitable vaccine delivery, and addressing immunization gaps to achieve sustainable health outcomes.

POSITION OVERVIEW

The Lady Health Visitor (LHV) will provide maternal, newborn, and reproductive health services through mobile outreach, focusing on antenatal and postnatal care, family planning, immunization, and basic treatment of common ailments. Acting as a bridge between the community and formal healthcare, the LHV will deliver WHO-aligned, evidence-based care, counsel families to promote health-seeking behaviors, and ensure referrals to government facilities for high-risk cases. This role is key to improving health outcomes, increasing immunization coverage, and strengthening primary healthcare access in underserved areas.

RESPONSIBILITIES

DESCRIPTION

1.  ANC screening (Antenatal Client)

  • Provide evidence-based ANC services in line with WHO guidelines, ensuring a minimum of eight ANC visits per client (or as feasible in the outreach context).
  •  Perform routine ANC assessments, including:
  • Screen for danger signs (e.g., severe anemia, gestational hypertension, pre-eclampsia, diabetes, hyperemesis gravidarum).
  • ·Administer tetanus toxoid (TT) vaccinations according to WHO/EPI schedule.
  •  Distribute iron and folic acid supplements (IFA) and educate on adherence to prevent anemia.
  • Provide dietary counseling to improve maternal nutrition and reduce the risk of complications.

2. PNC screeninge (Postnatal Client)

  • Provide WHO-recommended PNC visits within 24 hours, 7 days, and 6 weeks of delivery to assess maternal and neonatal health.
  •  Monitor for postpartum complications such as excessive bleeding, infection, or depression.
  • Counsel mothers on exclusive breastfeeding for the first six months and infant nutrition practices.
  • Ensure maternal immunization, including TT booster doses, if due.
  • Identify danger signs in newborns (e.g., jaundice, fever, poor feeding) and refer to higher-level facilities if needed.

3.  Family Planning:

  • Offer WHO-approved family planning methods, ensuring client choice, privacy, and informed consent.
  •  Provide and counsel on modern contraceptive methods, including:
  •  Address myths and misconceptions about family planning through culturally sensitive counseling.
  •   Maintain contraceptive supply chain and report stock levels to ensure continuous availability.

4. Management of minor ailments

  •  Provide diagnosis and basic treatment for minor ailments including but not limited to:
  •  Urinary tract infections (UTIs): Educate on hydration, hygiene, and prescribe antibiotics if required.
  • Hyperemesis gravidarum: Provide rehydration (oral or IV), antiemetics, and dietary advice.
  • Refer clients with severe or unresolved symptoms (e.g., fever, sepsis, or complications) to the nearest government health facility for advanced care.

5.Maternal, Newborn, and Child Health (MNCH) Advocacy

  • Conduct community education sessions to promote:
  • The importance of routine ANC/PNC visits.
  •  Immunization for both mothers (e.g., TT vaccine) and children (EPI vaccines).
  • Danger sign recognition in pregnancy and postpartum periods.
  • Safe delivery practices and institutional deliveries.

6.     Record-Keeping and Reporting

  • Maintain accurate, records, including:
  •  ANC/PNC registers.
  • Family planning client cards.
  • TT immunization records.
  • Submit daily, weekly, and monthly reports to the Mobile Outreach Team Lead for monitoring and evaluation.

7.     Referral and Follow-up Mechanism

  • Establish and strengthen referral pathways to nearest government health facilities.
  • Provide follow-up support to ensure adherence to treatment or care plans and continuity of care.
  • Collaborate with other mobile outreach staff (e.g., vaccinators, community outreach workers) to ensure holistic service delivery.
