• Work in close collaboration with partners to assist management team to coordinate the development of relevant work plans for effective implementation of HFCP.
• Support THF Team for the implementation of the approved plans.
• Monitor field based interventions to give immediate feedback to supervisor with aim to ensure timely remedial measures.
• Support planned advocacy/ social mobilization activities with innovative approaches, particularly engaging district authorities, schools, LHWs, Community leaders, district media.
• Identify and build new partnerships for HFCP at the field level-private sector, education sector, NGOs, etc.
• Jointly review field data of the town and develop plan of action for ongoing data-driven service delivery improvements as well as community mobilization.
• Develop a monthly work plan and submit to the supervisor.
• Facilitate the supervisor in regular briefings to the executive board/donor on key issues/bottlenecks of HFCP.
• Assist and facilitate relevant training activities for target groups.
• Prepare monthly progress reports and share with the supervisor (STC) and the provincial focal points.
• Develop quarterly report and share with the supervisor for further submission to executive board/donor.
• Ensure effective utilization of funds/resources provided and submit utilization report.
• Prepare end of year consolidate report.
• Any other assignment given by supervisor.

Leading and Supervising for Social Mobilization
Support Social Mobilizers in creating awareness and educating the community about ongoing activities for polio eradication and routine immunization.
Identify, recruit, train (build capacity of Social Mobilizers on skills, effective counseling such as partnership building. Planning etc) and supervise social Mobilizers in high risk areas.
Facilitate SM induction trainings as well as Refresher trainings with the help of Line managers.
Provide exhaustive field orientation and on-the-job training to social Mobilizers.
Relating and Networking (Community engagement)
Identify non-government organizations (NGOs) and civil society organizations (CSOs) who can conduct social mobilization activities at the community level and conduct resource mapping.
Plan and co-ordinate activities with NGOs working for social mobilization
Develop partnership with the Medical Officers MOs/Executive District Officer (EDO)/Supervisors/Health workers to plan and implement social mobilization/Information Education Communication (IEC) activities for the polio rounds and routine immunization.
Planning and Organizing
Tracking and mapping of highest risk areas for communication activities.
Identify information education communication requirements and distribution plan and coordinate with necessary partners to ensure implementation.
Assist in preparation of SM work plan and IEC activity plan for the respective areas.
Support DPEC meetings in developing and implementing social mobilization plan.
Implementing & Monitoring
Monitoring effectiveness of SMs and the activities conducted by SM.
Ensure orientation of Religious leaders and community leaders.
Conduct meetings and events with leaders before each round to motivate and mobilize them.
Support Government partners during Polio round to cover missed households.

Weekly feedback to DHCSO on progress, constraints and performance.

• To facilitate Union Council Medical Officer (UCMO) and Area Incharges/Vaccinators regarding SIAs Activities, NIDs/SNIDs, SIADs MOP UP and routine immunization.
• Assist in preparation and updating SIA micro planning as per ANEAP indicators and mapping high risk groups, scattered areas and hard to reach jurisdictions.
• Help in identifying and mapping high risk groups in the specific areas.
• Assist in selection of appropriate vaccinators as per given criteria and in their distribution as per need.
• To facilitate the UCMO & UC Secretary to arrange a fruitful UPEC meeting at the UC level and ensure 100% participation of UPEC members, submission and revision of Micro plan.
• Assist the PEOs and district trainers in planning, coordinating, and organizing training sessions for vaccination teams and supervisors.
• Monitoring the process of implementation of the campaign as per guideline and provide feedback to PEO.
• Data collection and analysis tally sheets from the area of responsibility on daily basis.
• Ensure proper implementation of SIAs with special attention to hard-to-reach areas and under- served high risk populations.
• Participate in preparation of realistic catch up/re-do sweeping activities, and facilitate to the work of independent monitors.
• To conduct Market Survey 5th day after campaign especially at Public Places, Main Entrances, Doctors areas and high risk groups, Mobile/Migratory populations.
• Supervise and coordinate field staff in interacting actively with primary stakeholders to learn how to improve implementation on a continual basis.
• Participate in the development, regular revision and updating of the information system so progress with the project component can be monitored, problems identified and its impact assessed accurately.
• Identify what support is needed to undertake good M&E of the component, and ensure this support is forthcoming.
• To mobilize relevant M&E technical assistance in a timely manner, with clearly demarcated responsibilities that are based on the participatory and equity principles of the project.
• Regularly appraise staff and provide feedback and support to enable them to do their jobs better.
• Encourage staff to report frankly on fieldwork, highlighting problems and possible solutions plus lessons learned. Reward innovation in critical reflection and learning.
• Assist in designing and implementing participatory planning processes and procedures.
• Occasionally assist with the work of short-term consultants, facilitating their access to the information they require.
