Support the development and implementation of a sustainable promotion program in the targeted communities and Health facilities:
Identify the areas of need for health promotion and education within targeted communities through contact with community representatives and assessment of knowledge and practices amongst clinics attendants.
Conduct trainings of community on mother and child health services and hygiene promotion and the implementation of different techniques & activities in selected communities.
Promote utilization of services available at nearby health facilities by monitoring and supporting the activities.
Identify the beneficiaries and refer to DHQ
Collection of beneficiaries data on daily basis and share with DC
Any other task assigned by the Coordinator

Outreach officer
Beneficiary anthropometric measurement and record-keeping
Conduct quality anthropometric measures at the sites on the referral beneficiaries following the initial screening exercise while ensuring efficient and effective workflow.
Compile, organize and maintain accurate records of beneficiaries referred to by documenting all the essential information in a hard-copy according to the standard of recording format.
Problem solve of beneficiaries complaint related to records and admission criteria.
To counsel all patients for further follow ups through Community Out Reach officer.
Work with the outreach teams to develop and maintain a referral system at community level.
Engage LHWs in referring malnourished children and pregnant and lactating women to Ehsas Nashonuma center.
Deliver awareness sessions on Mother and child health .
Participate in the development of fact sheets and other training materials as required for the MOH health staffs.
Assist the OTP/SFP Supervisor in providing MOH staffs with technical support in convening meetings and answering questions on topics relevant to the project.
Collaborate with health facility Staff for the day-to-day implementation of the EN services by developing new measures and/or maintain existing measures as directed.
Identification of Case Studies and writing up success stories
Check the condition of anthropometric materials received or to be sent, and verify the accuracy of any previously inventory of the tools
Responsible for proper handling of weighing and measuring equipment’s. Ensure scales are properly calibrated at all times and report defects, damage or loss of equipment to the Coordinator.
Attend regularly scheduled staff meeting at the office as directed by the Manager.
Establish a good understanding of the program within the community.
Ensure active screening of targeted beneficiaries through LHWs, CMWs and MOTs.
Nutrition Assistant
Mobilize, coordinate and monitor nutrition activities of healthcare providers (LHVs/FMTs) and Health Workers (LHWs/LHSs) and partner organization implementing nutrition project in the target areas.
Undertake at least 3 field visits in a week on a regular basis and duly report to the District Department of Health and Provincial Program Coordinator on findings and develop strategies and recommendations improvement in the nutrition programs
