Responsible for CMAM community outreach component activities, establishment community nutrition sites to ensure proper screening.MM supplementation create effective referral system.Implementation of SBCC on health nutrition awareness and community support group capacity building under the supervision of Nutrition Officer.Individual accountabilities.Responsibility to ensure the effective implementation of CMAM community outreach activities in the field. Develop micro plan of community outreach activities and submit on monthly basis. Community sensitization on the program objectives and methods to stimulate & eventually promote the understanding and participation of the target population. Establishment of community outreach setup at each union council through selection of community volunteers and health workers. beneficiary screening, provision of MM Supplementation and follow up. Establishment of strong referral mechanism between community to health facility OTP/CMAMI site and follow up. Support to the Community Health Worker (CHW) and Volunteers during screening (active case finding) and linking up the MTMSG, FTFSGs with the LHW program. Provide technical support to CHW on mobilization, active case finding, defaulter tracing and home follow up visits through follow up trainings and on job coaching and supportive supervision. Ensure community social behavior change communication health and awareness session/campaign on regular basis through CHW/support groups. Establish linked and meetings with the Community leaders and different group existing in the working area.
Nutritional Screening of all the children (6-59 months), pregnant women and lactating mothers.
Admit eligible children in SFP and OTP Program.
Maintain and update the record of the admitted children, pregnant women and lactating mothers.
Arrange, organize and conduct health, IYCF, hygiene and nutrition education sessions at the health facility.
Maintain and report the SFP /OTP supplies record properly at the respective health facility.
Ensure immunization of all the admitted children according to CMAM protocol and guidelines.
Systematic reporting of community nutrition screening, health & nutrition education sessions.
Actively participate in the improvement and strengthening of CMAM strategies and methodologies.
Submit the reports as required by the program and facilitate any other program concerned activity when needed.
Assist in the nutrition coordinator in monitoring and reporting of activities
Perform appropriate assessments of IYCF needs and malpractices and do analysis and come up with clear recommendations.
Develop action plan to improve IYCF practice as per national guideline and give technical guidance for the nutrition teams (Nutrition Assistant/Lady Health Visitor/Leady Health Worker/Community Resource Person)
Build the capacity of Outreach supervisor and Community Resource Persons on the community IYCF counseling techniques and ensure that the appropriate techniques are applied all the time.
Ensure formation of the mother support group at the community level(in each village) and community IYCF counseling conducted for the group members on regular basis.
Identify model mothers and explore their best experience and replicate the best experience in order to improve IYCF practice within the community.
Ensure setting up of breast feeding corners at each OTP/SFP sites established at Health facilities.
Provide appropriate counseling for PLW, care takers of OTP/SFP beneficiaries and mother groups on breast feeding ,complementary feeding and maternal and child nutrition.
Identify/develop appropriate IEC materials and flash cards in local language and ensure that the materials are used during each counseling.
Develop weekly and monthly plan towards accomplishment of the AWP and monitor the plan accordingly.
Document case studies regularly and share to the team member and to Nutrition Officer.
Produce monthly report regarding IYCF activities and submit to District Nutrition on time.
Any other relevant task given by Supervisor.
