As a Home Care Nurse, you need to offers direct patient care in the patient`s residence. Works closely with doctors, caregivers, and family members to develop care plans, administer treatments, monitor health, and ensure patients remain comfortable in their home setting
Key Responsibilities:
- Provide in-home nursing care, including administering medications, wound care, and other treatments as prescribed by a physician.
- Monitor the patient`s condition by assessing vital signs, symptoms, and physical health, and make necessary updates to care plans.
- Educate patients and families about health conditions, treatment plans, medications, and best practices for home healthcare.
- Assist patients with personal care activities such as bathing, grooming, and mobility, if needed.
- Coordinate with doctors, therapists, and other healthcare providers to ensure continuity of care.
- Provide emotional support and counseling to patients and their families.
- Ensure a safe and clean environment in the patient`s home.
- Train and educate family members or caregivers on best practices and care procedures.
