·         Prior to implementation of activities should have received orientation on the subject and project.
·         Plan, coordinate, supervise/monitor and implement/ report nutrition activities of the project) and develop communication network for this purpose in coordination with DHO/ DD Nutrition..
·         Responsible for the smooth execution of district level activities.
·         Support in creating awareness and educating the community about ongoing nutrition activities(IYCF, CMAM)
·         Build capacity of HCP’s on skills, effective counselling and supervise health care providers in the project targeted areas..
·         Provide exhaustive field orientation and on-the-job training to the health care providers.
·         Identification of health facilities to be declared as OTP Sites in the respective districts in consultation with the district health officer (s).
·         Ensure the conduction of district level meetings on monthly basis.
·         Support the provincial nutrition cell in assessing the health facilities and conduction of a training need assessment of health facility staff.
·         Keep effective liaison with the department of health (with national programs for family planning & primary healthcare, MNCH and EPI/PEI) and other line departments in the district for participatory planning, effective implementation and maximum impact of the nutrition interventions.
·         Collect, compile & analyze project data to monitor progress on monthly basis.
·         Share the collected data with the Information Management officer at Provincial Nutrition Cell and District Health Officer. 
·         Plan, develop and undertake need based trainings on nutrition interventions and provide technical assistance to targeted healthcare providers in the targeted health facilities..
·         Mobilize, coordinate and monitor nutrition activities of healthcare providers (LHVs) and Health Workers (LHWs/LHSs) and partner organizations implementing nutrition projects in the targeted areas.
·         Undertake field visits on weekly basis and duly report to the District Health Officer and Deputy Director Nutrition, on findings and generate recommendations for the improvement in the nutrition program.
·         Provide and submit weekly and monthly work plans and activities reports on regular basis, followed by a comprehensive report by end of the project.
·         Responsible for the receiving and further distribution of the nutrition supplies to the health facilities in coordination with district health officer.
·         Monitor activities of nutrition program in the district, provide feedback and facilitate them in smooth implementation of planned activities.
·         Coordinates with the provincial and district (DHO/DHMT) on progress and other development.
·         Any other task assigned by the supervisor

Responsibility:
The Social Mobilizer reports directly to the Project Coordinator PC and is responsible for the implementation of LRF Project

Plan, organize and manage all of the activities related to the Community mobilization, including serving as the contact Person to the Community and village leaders.
Community sensitization on the program objectives and methods to stimulate & eventually promote the understanding and participation of the target population. Community sensitization starts at village- and Community-leader levels and eventually extends to the entire population.
Elicit community involvement in case finding, referral, encouraging attendance and minimize defaulters. Follow-up issues related to low attendance rate and defaulters and the underlying problem and propose appropriate solution
Support to the Community Nutrition Volunteers (CNVs) during screening (active case finding) and linking up the CNVs with the LHW program.
Prepare activity plan for the screening exercise in consultation with the CNVs and OTP Supervisors.
Seek and provide feedback on screening programmed implementation schedule and activities plan as appropriate to enhance coverage and maximize impact
Participate in the selection of the decentralized mass screening sites to ensure access and high coverage
Organize and facilitate regular community screening sessions for CNVs with the guidance of OTP Supervisors.
Provide technical support to CNVs on mobilization, active case finding, defaulter tracing and home follow up visits through follow up trainings and on job coaching and supportive supervision.
