Descon Project @ Engro Polymer & Chemical Ltd, (Port Qasim, Karachi)Responsibilities are described as:• To conduct safety orientation to the new workers and maintain the record.• To conduct TBT for awareness about hazards at worksite and also discussed previousobservations.• Inspection of power and manual tools and PPEs.• Visit the worksite to verify safe workplace• Note down all work permits & observations and rectify them according to workpermits.• Inspection of scaffolding.• Implement all PPEs mentioned on work permits• Verify 3rd party certificates of crane, web sling, wire sling, crane operator, shackles,man-basket, full body harness, fall arrestor before lifting operation.• Inspection of crane including checklist, fire extinguisher, spark arrestor, SLI workingcondition, display of load chart, safety latch on hook,• Maintain all documents including safety orientation, TBT, near-misses, First aid case,MTC, LTI etc.

Descon shutdown @ National Refinery Limited (Karachi)Responsibilities are described as:• Check out all work permits before start of jobs• Monitor and rectify all unsafe act and unsafe conditions during shutdown activity• Check out the arrangements for hot jobs like canopy, fire watcher, fire extinguisher,fire hydrant hose pipe, grinder dead man switch, power tools inspection.• Check out confined space protocols like SBM (or hole watcher), log sheet, lifeline, fullbody harness, gin-wheel pulley, cross ventilation, light bulb, regular breaks.• Inspection of scaffolding.• Ensure barrication and latching of lanyard hook during work at height.• Check out third party certification of crane and crane operator and riggers before crane lifting operation.

Responsibilities are described as:• Evaluation of company's practices, processes, and procedures with respect toISO 9001, 14001, 45001 Standards.• To check all permit to work (PTW) at work sites.• To conduct Tool Box Talks (TBT) every day before starting the job in the workshop.• To monitor incidents of injury, illness and unsafe condition within an assigned area.• Stop any unsafe acts or processes that seem dangerous or unhealthy.• To maintain HSE documentations like Hot Work Permits, Cold Work Permits,Minutes of Meeting(MOM), HSE Induction trainings, Work at height certificates,confinedspace certificates, training sessions etc.• Ensure that all the activities in the workshop are conducted under hot work permit andcold work permit.• Assess risks associated with tools and equipment, jobsites, and work environments.• Inspection of welding plants, LPG cylinders, scaffolding, work at height, liftingoperations.• Inspection of all fire buckets and fire extinguishers at workplace.• Monitoring the unsafe act in the workshop and then correct the unsafe act.• Ensure that all the basic needs (water, rest breaks, PPE’s) are provided to the workers.• Ensure housekeeping and HSE standards in the workshop.• Ensure that all the personnel wear PPE’s at workplace.• Note down all observations and near-misses to prevent from future accidents.• Provision of First Aid treatment to the injured person immediately.• Ensure that all the inlet & outlet pipes of Gas Cutting machines are not damaged and jointless.• Checking the functioning of Flash-back-arrester during gas cutting.
• Ensure JSA (Job Safety Analysis) and TRA (Task Risk Analysis) in each operation inthe workshop.• Develop Emergency Response Plan (ERP) at worksites.• Ensure First Aid Arrangements in the workshop.
