Objective:  This position is responsible for overseeing all healthcare activity and implementing the HDF Health Program in the Region as per HDF Health Policy.

Key Responsibilities:

  • Health Coordinator (HC) will ensure implementation of all activities /Tasks of the health program at the Community Health Center (CHC) and field are aligned with HDF policies, SOPs, and Standards of quality.
  • Support RPM in preparing DIP of AOP.
  • Ensure smooth implementation of AOP (Health).
  • Examination of outdoor patients at CHC and Mobile Health Units/ Medical Camps.
  •    Responsible for bookkeeping/record and data management.
  • Referral of high-risk patients to nearby appropriate health facilities.
  • HC will ensure the
  • Daily visits of CHWs.
  • Scheduled visits of LHV.
  • Monitoring visits to the program area to assess the performance of LHV and CHWs in the field.
  • HC will visit the field ONCE a week.
  • Implementation of School Health Program.
  • To facilitate LHV in record /data keeping as per standards.

HC will ensure

  •  Immunization of all Preparation of demand/requisition for medical supplies.Maintenance of Health Fund.Responsible for implementation of Clinical governance in CHC.
  • Implementation of infection prevention and infection control policies, SOPs, and guidelines.
  • With the help of RPM and the health team, she/he will ensure the implementation of Hospital Waste Management. (HWM)Preparation, 
  • Analysis & correction & time data & progress sharing on & monthly basis in monthly, quarterly, and annual progress reports.
  • Generation of field monitoring report on specified Formats.Liaison/linkage development with the Govt health Department.
  • Liaison/linkage with other like-minded NGOs working in the same region/area.Maintenance of correspondence record with National Office, RegionsAny other official task assigned by Supervisor (s)
