The Clinical Pharmacist will be responsible for providing clinical pharmacy services to patients, including point-of-care pharmacy rounds with multidisciplinary teams, review of patient profiles, identification and rectification of drug-related problems, development and implementation of clinical and medication safety guidelines, and collaboration with healthcare professionals to address medication therapy-related queries.

Responsibilities:

  • Compiles data and updates drug information software regularly.
  • Handles drug or poison-related queries and maintains records.
  • Utilizes primary, secondary, and tertiary drug information resources.
  • Runs clinical indicators such as therapeutic drug monitoring and special dose adjustments.
  • Performs antibiotic stewardship activities and maintains records.
  • Prepares patient profiles, interprets lab results, and identifies drug-related problems.
  • Conducts clinical rounds with doctors and records interventions.
  • Reviews prescriptions for appropriateness, charges, labels, and dispenses medications.
  • Compiles adverse drug reaction (ADRs) and trigger tool data.
  • Prepares drug evaluation monographs (DEMs) and other data required for the Pharmacy & Therapeutics Committee.
  • Assists in developing and releasing pharmacy updates, memos, alerts, and publications.
  • Conducts continuous education and professional development sessions internally for staff, Interns and Pharmacy Residents and for external audiences
  • Orients and facilitates the training of under-training pharmacy staff.
  • Follows departmental policies, procedures, and standard operating procedures.
  • Adheres to infection control practices, safe patient care, and confidentiality.
  • Respects cultural values and demonstrates sensitivity.
